Please read the following and check the box next to each statement to signify that you have read and understand it.*

I understand that submitting this 30 day notice does not make it official and legally binding until HPM acknowledges receipt and approval of it.

I understand that my notice must be given no later than the 1st day of the the last month of my lease term. I also understand my notice must end on the last day of the month of my lease term, that HPM does not pro-rate rent and allow mid-month move outs and that I will still be required to pay rent for the entire final month but I may vacate early at any time during that final month.

I understand that once notice has been delivered to, acknowledged, and accepted by HPM, my vacate on or before date will be the expiration date listed on my lease unless otherwise agreed to in advance in writing by both parties. No additional time to vacate will be given, including allowing a month to month tenancy.

I understand that I must provide HPM with my forwarding address on or before my vacate date or risk HPM mailing my security deposit disposition letter and security deposit refund check (if applicable) to my current address which may delay delivery due to the USPS having to forward mail to my new address.

I understand that per Kansas Landlord Tenant Law, that I am required to return the unit to HPM in equal or better condition than it was given to me minus normal wear and tear or HPM can deduct necessary repairs and cleaning that I am responsible for from my security deposit.

I understand that the carpets in my unit must be professionally cleaned at my expense and a receipt provided to HPM on or before my vacate date or I will be charged by HPM out of my security deposit to have the carpets professionally cleaned.

I understand that I am not allowed to attempt to touchup the paint colors in my unit or patch any holes in the walls or I may be charged for repainting the entire unit due to paint color variations or correcting any attempts at wall repair out of my security deposit.

I understand that I am not to leave large amounts of trash on the curb as I leave the unit for the final time and that the cost of hauling off any trash that my trash service doesn’t haul off will be deducted from my security deposit.
